How it works:

Print out and bring the most recent version of Form I-693, Report of Medical Examination and Vaccination Record to your appointment.
Complete Part 1 of Form I-693 but do not sign until Dr. McCarthy instructs you to do so. We’ll use this form to document your exam. You can also fill out your name at the top of each page (14pgs!) as this saves us time!!
Bring your vaccine record and any pertinent medical history records.
Bring an interpreter with you to your visit if necessary!
Your visit will include the actual physical exam, screening for communicable diseases and a review of your vaccines. At the end of your visit you’ll be given an order for the required labs (these ARE included in the cost of the exam!) and guidance regarding any vaccines you might need or specialist you will need to see.
You’ll get your lab work drawn through our partner lab in Christiansburg, VA. As soon as results are available (typically less than 1 week) Dr. McCarthy will contact you; if all is well you can come by the office to pick up copies of your form (one for you and one in a sealed envelope!) If further care is needed we’ll talk you through the steps that need to be completed before Dr. McCarthy can sign the form.
Total cost is $400. This includes the labwork!
Vaccines are offered through a partner pharmacy at discounted prices.
Once we get you scheduled we can usually get everything done within one week!
